It is vital that any and all changes (including cancellations without a reschedule date) is submitted. In order for the Heart to review requests to change all scheduled varsity contests (conference and non-conference), this form must be submitted by the host(s) institution. It is required that the host Athletic Director submit the request. It is assumed by the Heart office that all schools affected by the request have been communicated with and agree to the proposed changes. In the case of multiple affected schools/hosts, one AD may submit the form on behalf of all schools.
This form should be used for ALL changes.
Submitting the request to change a scheduled contest to a different date does not guarantee approval. You will receive communication of approval, denial, or the need for further clarification from the Heart office.
