Schedule Change Request

In order for the Heart to review requests to change all scheduled varsity contests (conference and non-conference), this form must be submitted by the host(s) institution. It is required that the host Athletic Director submit the request. It is assumed by the Heart office that all schools affected by the request have been communicated with and agree to the proposed changes. In the case of multiple affected schools/hosts, one AD may submit the form on behalf of all schools.

This form should be used for changes that are more than 24 hours in the future. In the case of emergency change requests or within a 24-hour window, contact the affected schools and the Heart office via email or phone.

Submitting the request to change does not guarantee approval. You will receive communication of approval, denial, or the need for further clarification from the Heart office.
